Snake Envenoming Management (SEM) Program Overview

Introduction

Snakebite envenoming is a significant yet often overlooked public health issue in Malaysia. This neglect has inadvertently led to widespread inappropriate clinical practices, contributing to suboptimal patient outcomes. Recognizing the urgent need to address this gap, a group of dedicated Emergency Physicians with a special interest in Clinical Toxinology established the Remote Envenomation Consultation Services (RECS). These consultants, who are also active members of the Malaysian Society on Toxinology (MST) and the College of Emergency Physician Special Interest Group on Clinical Toxinology (ClinToxin SIG), provide expert guidance to ensure optimal clinical management for envenomed patients across Malaysia.

Through their work, RECS identified a critical lack of knowledge and confidence among healthcare providers regarding proper snakebite management. Many healthcare professionals rely on outdated or erroneous references, leading to poor clinical decisions and treatment outcomes. In response to this, RECS developed the Snake Envenoming Management (SEM) program—an intermediate-level educational initiative designed to equip healthcare providers with the knowledge and skills needed for effective snakebite management.

SEM was first introduced in 2010 as a pre-conference workshop during the Biennial Scientific Meeting of the Postgraduate Medical Education Society (PGMES) at Hospital Sultanah Bahiyah, Alor Setar, Kedah. Since its inception, SEM has served as a dynamic platform for healthcare providers to share knowledge, experiences, and best practices. With its interactive, open, and guided approach, SEM fosters adult learning in a supportive environment, empowering participants with confidence and competence in treating snakebite envenoming cases.


General Objective

To enhance the knowledge, skills, and confidence of healthcare providers in managing snakebite envenoming, ensuring patients receive appropriate care for optimal outcomes.


Specific Objectives

  1. Recognize Snakes of Medical Significance:
    Introduce participants to venomous snake species in Malaysia and their primary venom effects.

  2. First Aid for Snakebite:
    Provide evidence-based first aid practices for snakebites in prehospital and hospital settings.

  3. Clinical Management & Antivenom Use:
    Present appropriate clinical management strategies, including the judicious use of antivenom in Malaysian snakebite cases.

  4. Guidelines for Snakebite Management:
    Familiarize participants with up-to-date and relevant snakebite management guidelines tailored to Malaysia's healthcare system.

  5. Raise Awareness of RECS Services:
    Increase awareness of the Remote Envenomation Consultation Services (RECS) and its role in supporting healthcare providers nationwide.


Program Format

SEM is a one-day workshop/seminar designed for healthcare providers, including doctors, nurses, and pharmacists. The program is highly interactive and includes case discussions, expert lectures, and hands-on learning sessions. It is conducted several times a year, based on requests from healthcare institutions and organizations.
